Does Medi-Cal Cover Braces? Understanding the Orthodontic Medi-Cal Process and Your Options

One of the most common questions we hear at Life Orthodontics is: "Will Medi-Cal cover braces for my child?"

The answer is sometimes, but only if very specific medical criteria are met. Many parents are surprised to learn that most children with Medi-Cal do not qualify for orthodontic treatment, even if their teeth appear crowded or crooked. Medi-Cal only covers orthodontic treatment when it is considered medically necessary, not simply because braces would improve the appearance of a smile.

This guide explains how the Medi-Cal orthodontic approval process works, who qualifies, and what your options are if your child does not qualify.

Who Can Receive Orthodontic Coverage Through Medi-Cal?

Orthodontic treatment through California Medi-Cal is generally available only for patients under 21 years of age who meet the state's medical necessity requirements.

Once a patient turns 21, orthodontic treatment is generally no longer covered by Medi-Cal, regardless of how severe the orthodontic problem may be.

How Does Medi-Cal Decide Who Qualifies?

There are three primary pathways for approval:

1. The Handicapping Labio-Lingual Deviation (HLD) Index
2. Automatic qualifying conditions
3. Medical necessity through an EPSDT request

Let's take a closer look at each.

Pathway 1: The HLD (Handicapping Labio-Lingual Deviation) Index

The most common method for determining eligibility is the HLD Index, a standardized scoring system used throughout California to evaluate the severity of orthodontic problems.

During your orthodontic evaluation, we measure several aspects of your child's bite and tooth positions, including items such as crowding, spacing, overjet, open bite, missing teeth, impacted teeth, and other significant bite discrepancies.

Each finding is assigned a certain number of points. To qualify under this pathway, a patient must receive an HLD score of at least 26 points.

Many children have orthodontic problems that would clearly benefit from braces but simply do not accumulate enough points to reach the required score. This is one of the primary reasons most Medi-Cal patients are ultimately denied orthodontic coverage.

Pathway 2: Automatic Qualifying Conditions

Certain severe orthodontic conditions automatically qualify for Medi-Cal coverage regardless of the HLD score. These include cleft lip and/or cleft palate, craniofacial anomalies with appropriate specialist documentation, a deep impinging overbite causing the lower front teeth to damage the soft tissue behind the upper front teeth, an anterior crossbite causing destruction of soft tissue, an overjet greater than 9 mm, a reverse overjet greater than 3.5 mm, and severe traumatic deviations involving significant injury or pathology affecting the jaws or teeth.

These conditions are relatively uncommon, which is another reason why only a small percentage of Medi-Cal patients qualify for orthodontic treatment.

Pathway 3: EPSDT (Early and Periodic Screening, Diagnostic and Treatment)

Even if a patient does not reach 26 HLD points or have one of the automatic qualifying conditions, orthodontic treatment may still qualify under EPSDT, which stands for Early and Periodic Screening, Diagnostic and Treatment.

EPSDT allows medically necessary treatment when orthodontics is needed to improve another medical condition or functional problem. This generally requires a detailed letter explaining why orthodontic treatment is medically necessary.

Examples may include significant speech difficulties, chewing problems, and other medically significant functional concerns. Supporting documentation from physicians, speech-language pathologists, or other specialists may also be required depending on the situation.

EPSDT requests are reviewed individually, and approval is never guaranteed.

Why Most Children Do Not Qualify

Many parents are understandably disappointed after learning their child does not qualify for Medi-Cal orthodontic coverage.

The reason is that Medi-Cal only covers severe orthodontic problems that meet strict medical necessity criteria. Many children have crowded teeth, gaps between teeth, mild overbites, mild underbites, or moderate bite problems. These conditions often benefit tremendously from orthodontic treatment, but they simply do not meet Medi-Cal's requirements for coverage.

The system is largely all or nothing. A child who scores 25 points receives no orthodontic benefit, while a child who scores 26 points qualifies for treatment.

What About Phase I Orthodontic Treatment?

Parents are often referred for Phase I orthodontic treatment, also called early interceptive treatment, between ages 7 and 10.

While Phase I treatment can be extremely beneficial in the right situations, most Phase I cases do not qualify for Medi-Cal coverage. The exceptions are typically children who meet one of the automatic qualifying conditions, or who qualify through an EPSDT request with appropriate supporting documentation.

This is because many Phase I treatments are intended to guide growth, prevent future problems, or simplify later treatment rather than correct a severe existing medical condition.

What Happens During a Medi-Cal Orthodontic Evaluation?

At Life Orthodontics, we perform a comprehensive orthodontic evaluation that includes a thorough clinical examination, digital photographs, digital scans, bite measurements, HLD scoring, evaluation for automatic qualifying conditions, and a review of whether an EPSDT request may be appropriate.

If it appears your child may qualify, we can explain the documentation that would typically be required for submission. Ultimately, Medi-Cal makes the final decision regarding approval.

What If My Child Doesn't Qualify?

Not qualifying for Medi-Cal does not mean orthodontic treatment is unnecessary.

Many children who do not qualify still benefit tremendously from orthodontic treatment because it can improve oral hygiene, correct crowding, improve bite function, reduce uneven tooth wear, improve long-term dental health, and increase confidence and self-esteem.

Some of the healthiest, most stable smiles we create are for children who never met Medi-Cal's strict qualification requirements.

Life Orthodontics does not participate with Medi-Cal for orthodontic treatment. However, we understand that this creates a difficult situation for many families, especially since most children with Medi-Cal ultimately do not qualify for orthodontic coverage.
